New Patients

During your first visit to Bandon Coastal Dental, our objective is to get to know you better, to become familiar with your past dental experiences and to address any current concerns you may have.  We are sensitive to your needs, and it is our goal to make you feel comfortable visiting our practice while providing the best care possible.

Please be prepared to complete all insurance and health information forms.   To expedite this process, you may print the necessary forms below, fill out and bring with you to your first appointment.

If you cannot make an appointment or may be late, we ask that you notify us as soon as possible so that we may offer that appointment to another patient.  A 24 hour notice is requested.
Our office will contact you 24 hours prior to your appointment as a friendly reminder.  Please provide us with as much contact information as possible so that we may reach you.

EMERGENCY CARE

If you or a family member are a current patient of record with our office and you develop an emergency situation after hours, please contact our office and follow the instructions on how to contact the doctor. If you are unable to reach the doctor in a timely manner, please consult the Emergency Room as necessary.
